Don’t go for keto diet before reading this

written by Dr. Basudev Tewari |     April 30, 2020

Keto diet is a very common weight-loss diet. But is it safe for all? Read this blog to know if you are able to try ketosis.

But again keto diet not just another fad diet! People normally try this using the internet Gyan! Remember, blogs and YouTube are not always enough. Keto diet has direct relationship with your body physiology. You can’t go for ketosis, always.

It’s been there for ages. From the 1920s we are using it for epilepsy. It became popular in weight loss in the 90s. Lots of research are nowadays going on keto diet. It’s not just a fad diet that will pass on. It’s there to stay. It has a scientific base.

Here total carb is below 5%, protein is moderate, around 25-30% (bit higher initially) and fat is high, almost 65-70%. But this high fat won’t harm you at all. Even the high saturated fat going in won’t clog in your arteries if you are on calorie deficit and doing regular exercise. Read my blog on keto diet myths, to learn more.

People are afraid of the low carbs too. They think lower carbs can decrease the exercise potential and may cause weakness and dizziness. That’s not the case. I have discussed that in the above blog. But yes, dehydration and electrolyte imbalance can cause some issues and you may feel weak and dizzy. This may be even more common during the initial keto flu

But keto is not for everybody. There are a few absolute contraindications (where you just cannot try ketosis). Also there are a few relative contraindications (where you can try ketosis but be cautious about it). But I shall recommend you to avoid it in even relative contraindications too. There are several others ways to lose weight.

Now let’s discuss them one by one…

Absolute contraindications (some uncommon genetic disorders) :

  • Carnitine deficiency (primary)
  • Carnitinepalmitoyltransferase (CPT) I or II deficiency
  • Carnitinetranslocase deficiency b-oxidation defects
  • Medium-chain acyl dehydrogenase deficiency (MCAD)
  • Long-chain acyl dehydrogenase deficiency (LCAD)
  • Short-chain acyl dehydrogenase deficiency (SCAD)
  • Long-chain 3-hydroxyacyl-CoA deficiency
  • Medium-chain 3-hydroxyacyl-CoA deficiency
  • Pyruvate carboxylase deficiency
  • Porphyria

These are all fancy names. Forget them if you don’t like to remember, lol. These are mostly inborn errors of metabolism but may develop late too. But if you have any of them, forget going keto!

Relative contraindications (extremely common – consult you doctor/physician, before starting keto diet) :

Type 1 diabetes mellitus- Type 1 diabetes or IDDM are from early age and they have destroyed beta cells of pancreas. So very little or no insulin is produced and patient needs external insulin support for life. It has a chance to develop runway ketosis as there is no insulin to stop it. Ketoacidos may be precipitated and that may be bad or even fatal.

Liver, gall bladder and pancreas problem – Even past history or family history can be bad. Keto is not allowed in hepatitis, cirrhosis, gall bladder stones or infections, pancreatitis or even any other problems with these organs. Even any history of gall bladder operation should discourage you from ketosis.

Kidney issues like kidney stone or kidney failure – Better avoid keto diet. Even any such history in family should raise red flags in your mind. Keto diet deals with lots of water and sodium/potassium excretion. Unhealthy kidneys can’t handle ketosis. Check kidney functions with BUN/creatinine level to decide.

Gastric issues like reflux, ulcer or gastritis – It’s better to avoid keto diet. Even gastric or gut surgery cases can’t go for ketosis. Also check for gut motility issues (few medicines, surgeries and diseases can cause it). Don’t also try it if you have severe constipation.

Poor nutrition and electrolyte imbalance –  A poor nutritional status of a person can be even worse for keto diet. Water, sodium and potassium is normally excreted in keto diet. These frequently coexist and can be a dangerous proposition to start keto diet.

Some mental problems – A few examples like depression, anxiety/stress, carb craving etc. can be bad for keto diet. Keto diet needs mental stability to continue and binging can’t be an option. It’s better to get to a standard, non ketogenic, low carb diet, in that case.

Conclusion : If you don’t have any above contraindications, keto diet is a great option for you to lose weight. In type 2 diabetes, obesity, belly fat, metabolic syndrome etc., keto diet can be used. Lots of research is going on this diet and it’s getting popular day by day. Read my blog on : Important FAQs on ketosis to know more. My YouTube video on that is soon coming up too.
